Are you an expert problem solver with a focus on customer service? The Patient Care Operations Coordinator (PCOC) II is responsible for the management of home health patient schedules in collaboration with clinicians and/or home health aides and managers. The person assures timely scheduling and communication with patients and clinicians/aides, ensuring patient and interprofessional team satisfaction is achieved, and possesses extensive HomeHealth scheduling experience, is able to work across teams and collaborate across departments to problem solve scheduling issues. Finally, this individual serves as a preceptor and mentor to other team members, role modeling ChristianaCare behaviors and demonstrates the ability to support others to learn scheduling procedures. Our Patient Care Operations Coordinators orient 9am-5:30pm and then work 9am-5:30pm or 8:30am-5pm depending upon business needs. The successful candidate will work rotating weekends, rotating holidays, and an on call rotation of 3-4 days per month, on call being 4:30pm-8:30am. This individual will complete the following:

  • Patient scheduling
  • Information flow within HomeHealth
  • Assisting Supervisors with supervising staffing levels
  • Maintaining tracking systems
  • Precept newly hired caregivers

Requirements:

  • Scheduling and HomeHealth care experience preferred
  • Associate degree required, Bachelor's degree preferred
  • Proficiency in MS Teams, Outlook, Excel, etc.
  • Must be organized and detail oriented and possess excellent communication skills
